Documentation

  • When a slot becomes available, the office will inform you. You will have 36 hrs to get the following information to the admissions team:

    • Insurance information

    • Prescription Letter from a Physician/Psychologist

    • Diagnostic Assessment (conducted by a Psychologist or Developmental Pediatrician)

    This information is sent to the insurance provider for approval for assessment.

  • Once insurance has approved Assessment units, assessment paperwork will be sent out to caregivers to complete prior to the assessment date. The assessment paperwork may include but is not limited to an intake questionnaire, preference survey and rating scales. The assessment date will be scheduled within 1-2 weeks of receipt of paperwork.

 
 

Assessment

  • On the day of your assessment, the caregiver(s) and the child is required to be present. Please bring with you any additional relevant paperwork (e.g., child’s IEP, speech or occupational therapy evaluation, records from previous ABA provider, etc). An intake interview will be completed with the caregiver while a skills assessment is completed with the child. Please expect for the assessment to last 1-1.5 hours in length.

    We will be assessing the child’s current levels of functioning in multiple areas including but not limited to the following: Communication, Social, Adaptive, Motor, Behavior, and Academic. Assessment measures used vary based on the client's need and functioning level and may include one or more of the following: Verbal Behavior Milestones Assessment Placement Program (VB-MAPP), PEAK Assessment, Vineland Adaptive Behavior Rating Scales, Other Rating Scales, Practical Functional Analysis (PFA) and/or Functional Behavior Assessment (FBA). Assessments are re-administered every 6 months to measure progress and growth.

  • Following the assessment, a comprehensive assessment report is completed and an individualized treatment plan is generated to address the behavior excesses and skills deficits identified. The treatment plan will include skills targeted, methods of treatment, recommendation for number of hours of services per week in addition to parent training goals. Skills targeted may be across multiple areas including but not limited to the following: Communication, Social, Adaptive, Motor, Behavior, and Academic.

    The report and treatment plan takes approximately 10 business days to complete. The reports will then be sent to insurance providers for approval of treatment units. The approval time varies based on the insurance company. Once insurance approval is received, the caregiver will get a call about the start date.
